Judson Senior Living in Cleveland, Ohio is seeking a Director of Independent Living and Hospice. Currently hiring for a Full Time opportunity with our Judson Home Care and Hospice program.

*Pay Range from $38 - $45/hour (Based on Experience)*

What will I do in this role with Judson?

As a Director of Independent Living and Hospice, supporting our mission will include the following responsibilities:

General duties:

  • Monitor, assess, and inventory service needs, service options, resource utilization, and revenues and expenses, including any grant funding available to the program.
  • Work closely with the Sales and Marketing team on strategies to increase visibility of Hospice in the community
  • Maintain necessary standards for licensure and certification
  • Participate in strategic planning with Corporate Executive team.
  • Participate in appropriate State and National professional organizations and alliances
  • Responsible for building team and both appreciative and respectful of diversity challenges and opportunities.
  • Provides oversight of the nursing assessments and development of plan of care for home care clients.
  • Insures that the required documentation including developing, implementing and evaluating the care plan, as changes in condition occur and as required.
  • Oversight of care delivered by the home care aide and makes adjustments to plan of care as needed..
  • Provides oversight of the nursing staff responsible for assessing resident/community client appropriateness in current level of care, assesses and advises when other services may be appropriate.
  • Effective communication with residents and families. Actively listens to resident and family concerns and identifies solutions.
  • Promotes and participates in Quality Improvement initiatives
  • Initiates or participates in care conferences and maintains effective communication with interdisciplinary teams.
  • Provides oversight of home visits to prospective move-ins - for assessment and alignment with appropriate level of care.

Hospice duties:

  • Assures that hospice documentation is completed in accordance with Medicare rules and regulations and within the specified time frames.
  • Insures that ongoing certification and licensure for hospice is maintained.
  • Completes referral information, assessment and care plan for hospice patients and families.
  • Provides oversight to insure that hospice services are delivered in accordance with Medicare regulations and mandates and maintains the comfort and quality of life for those who are diagnosed with terminal diseases.
  • Effective communication with families. Actively listens to family concerns and identifies solutions.
  • Participates and provides oversight for bi-weekly IDG meeting.
  • Provides oversight for and participates in Quality Improvement initiatives and QAPI for hospice.

Other Duties:

  • Maintains responsibility for interviewing, hiring and termination of employees under supervision.
  • Performs 90 day evaluation and annual performance appraisals for all employees.
  • Identifies and initiates training for employees
  • Coordinates and conducts competency skills evaluation, testing and review for IL nurses.
  • Provides oversight of HHA supervisory visits by nursing in conjunction with the Director of Home Care.
  • Provides back-up on-call coverage after hours and is therefore required to carry pager and have access to a phone at all times.
  • Responds promptly to all pages resolving all clinical care situations in association with existing cases.
  • Prepares and submits monthly documentation for billing.
  • Maintains a record of acceptable attendance and punctuality. Observes all company rules and safety procedures. Other duties as assigned

What do I need for this role with Judson?

  • Ohio RN license with BSN preferred 5+ years Management experience
  • Excellent written/oral communication's skills
  • Business acumen/able to develop and closely monitor budgets/marketing plan/growth strategies/and other support infrastructure needs
  • Creative thinking, problem solving and strategic planning skills
  • Flexibility of schedule - some evenings/weekends
  • Enjoys and is able to multi-task
  • High energy/competitive and enjoys being challenged
  • Demonstrates commitment to wellness and health maintenance
  • Advanced user of information systems
  • Proven record of reliability, dependability, accuracy and attention to detail, excellent interpersonal and communication skills
  • Good assessment, analytical and problem-solving skills
  • Must have an understanding and acceptance of the philosophy of care of JHC
  • Managerial experience regarding hiring, discipline, performance appraisals, etc.
  • Proficiency on windows-based computer word processing and spreadsheet software is necessary
